Come by Echols County Health Department to get a head start on your child’s needed health forms for entry into a Georgia school for the first time! You can get a certified copy of your child’s birth certificate (if born anywhere in Georgia), have their immunizations caught up if needed or get a copy of their immunization record from GRITS, and/or have their Form 3300 completed (ear, eye, dental and nutrition screening).
